Forms

This section of our site provides information for the legal aid professional such as solicitors.  If you want to apply for legal aid, visit our getting legal help section. Only a solicitor can complete and submit a legal aid application to us. To make an application for legal aid you should therefore consult your solicitor.

We provide digital versions of some forms and some are available from the Board. Most forms have been replaced within our Legal Aid Online service. Detailed below are our current forms which are available in pdf format.  You can also read below information on how to use the forms and download them.

Legal Aid Online allows you to send applications and accounts through the internet. Click here to access our Legal Aid Online mandate forms.

About using these forms

You will require Adobe (Acrobat) Reader to use the forms this software is free and available from Adobe's web site. Some of the forms available are interactive, which let you type into the digital version to complete it. Some forms that are only available to let you print it and then complete it by hand, are clearly indicated as print only versions. To use the forms, you can open each form and then save it to your computer, or you can right-click on the form name and then save it to your computer. Our help section has more information about using these Forms or getting Adobe Reader.

If you use the interactive versions and wish to save the data you have typed into the form, you need to purchase Adobe Acrobat software. You can find out more details of this software and buy it on Adobe's website. Other software providers offer equivalent products.

If you get an error message when opening up a form it is probably because you do not have Adobe software or a current version of Adobe software, you can usually download this free from Adobe's site.

Some forms can only downloaded via the website, some can also be ordered from the Board - see above order form. Most have been included within our legal aid online service.
