Forms

This section of our site provides information for the legal aid professional such as solicitors.  If you want to apply for legal aid, visit our getting legal help section. Only a solicitor can complete and submit a legal aid application to us. To make an application for legal aid you should therefore consult your solicitor.

We provide digital versions of most applications and accounts forms that are available from our printers. Detailed below are our current forms which are available in pdf format.  You can also read below information on how to use the forms and download them.

Legal Aid Online allows you to send applications and accounts through the internet, and brings many benefits. If you are not yet using Legal Aid Online, read about the benefits and register now to get started using the service. We no longer provide pdf versions of advice and assistance forms - instead you should use our supplied printed forms or Legal Aid Online.

The latest changes are shown in pink.

About using these forms

You will require Adobe (Acrobat) Reader to use the forms this software is free and available from Adobe's web site. Nearly all of the forms available are interactive, which let you type into the digital version to complete it. Those forms that are only available to let you print it and then complete it by hand, are clearly indicated as print only versions. To use the forms, you can open each form and then save it to your computer, or you can right-click on the form name and then save it to your computer. Our help section has more information about using these Forms or getting Adobe Reader.

If you use the interactive versions and wish to save the data you have typed into the form, you need to purchase Adobe Acrobat software. You can find out more details of this software and buy it on Adobe's website. Other software providers offer equivalent products.

All Board services will be available through Legal Aid Online, which will reduce bureaucracy and make the legal aid system quicker and more cost effective. Read more about the project and the benefits.

If you wish to order printed copies of these forms, contact our printers Stewarts on tel 0131 659 6010 or fax or mail an the above order form to them.
