Requesting copies of Form 2

Tuesday, Oct 20, 2015

As part of the ongoing development of the Form 2 for financial assessment of applications for civil legal aid we agreed with the Law Society of Scotland’s Civil Legal Aid Negotiating Team to review the way these forms are provided to applicants.

Your clients may have different preferences. For those who want to complete a paper form in writing, we have put in place temporary arrangements so you can request printed copies be sent to your office.

For those applicants who want to complete the form electronically, the Form 2 will continue to be available by download. Go to the forms page of our website and select CIV/FIN/2. This is the link you should give your clients if you want to use the online version.

Major changes to Form 2 were made in July 2013 and since then we have continued to seek views from applicants, solicitors and other interested parties about how we can make the form better and easier to use while we develop an online option.

We are continuing to monitor the Form 2 return rate and will make further changes where appropriate to reduce the very high volumes of incomplete forms being returned.

For this reason, we will not send out large stocks of the form to avoid wastage when further changes are made.

We hope you find this arrangement beneficial. If you have any queries, please contact Joe Kelly, Head of Civil Legal Assistance - kellyjo@slab.org.uk

How to request Form 2 copies

If you require copies of the pre-printed forms, please email your request to communications@slab.org.uk, quoting form reference CIV/FIN/2 and providing your firm’s DX, LP or Royal Mail address.
